By the end of the session, participants will be able to:

  1. Describe enduring principles derived from family medicine’s roots in general practice

  2. Articulate the evolving nature of family medicine, from PCMH, to advanced primary care, to population management, teams, and partnership with public health

  3. Discuss current state of the evidence for effectiveness of advanced primary care
